When you arrive you will be asked to complete a patient registration form. We will also need to make copies of your insurance cards(s) and picture ID. It is very important that you provide us with accurate information and notify our office of any changes.
The Villages Clinical Laboratory is a “no appointment necessary” facility.
If you need a Glucose Tolerance test, you must call 352-751-8906 for an appointment. Glucose Tolerance testing is only performed in our main lab across the street from The Villages Regional Medical Center.
